3.5 Educating patients on the roles of health care team members; assisting patients and families in communicating with one another and to members of health care team; interpreting information;
3.6 Educating patients on the levels of health care (i.e.

- acute, home care); entitlements; community resources; and advance directives;
3.7 Facilitating decision making on behalf of patients and families;
3.8 Employing crisis Intervention;
3.9 Diagnosing underlying mental illness; providing or making referrals for individual, family, and group psychotherapy;
3.10 Educating hospital staff on patient psychosocial issues;
3.11 Promoting communication and collaboration among health care team members;
3.12 Coordinating patient discharge and continuity of care planning;
3.13 Promoting patient navigation services; occupational profile 1 Social Workers in Hospitals & Medical Centers
3.14 Arranging for resources/funds to finance medications, durable medical equipment, and other needed services;
3.15 Ensuring communication and understanding about post-hospital care among patient, family and health care team members;
3.16 Advocating for patient and family needs in different settings: inpatient, outpatient, home, and in the community; and
3.17 Championing the health care rights of patients through advocacy at the policy level.
