As part of our commitment to excellence, we are seeking a Remote Patient Monitor Specialist to join our team.
Qualifications: Previous experience in healthcare administration or a related field preferred. Familiarity with peritoneal dialysis (PD) procedures is a plus. Proficiency in using electronic medical records (EMR) systems. Excellent organizational skills and attention to detail. Strong communication and interpersonal skills. Ability to work independently and collaborate effectively with a remote team. Understanding of patient confidentiality and HIPAA regulations. Tasks required: Welcomes patients to the RPM program Educate patients on how to use the monitoring device Set up monitoring devices in the remote monitoring system. Tracks delivery of devices from the manufacturer to the patient's home. Reviews and ensures completion of clinical minutes and notes each month • Stores and transmits patient consent forms and adds consented patients to the software platform. Complies with all HIPAA guidelines and maintains the confidentiality of clients and patients Complies with Federal and respective state laws regulating health management organizations and telephone information centers and all department standards, policies, procedures, and licenses/certifications. Performs duties ONLY within the scope of practice approved by the state in which the patient is located. Provides utilization management and quality improvement in accordance with utilization policies and processes. Ensures adherence to the Quality Management Program. Performs other job-related duties as assigned Key of Responsibilities: Place patient orders accurately and efficiently, ensuring timely delivery of necessary supplies and medications. Chart patient notes in collaboration with nursing staff, documenting patient assessments, treatments, and outcomes. Conduct monthly audits of patient charts to ensure compliance with regulatory standards and internal protocols. Verify and update patient prescriptions to ensure they are current and accurate. Become proficient in our EMR system through training provided by the company. Upload and maintain patient flowsheets, ensuring accurate and up-to-date documentation of vital signs, lab results, and other relevant data. Ensure that care plans are completed and signed by appropriate healthcare providers. Provide support and guidance to nursing staff and other healthcare professionals as needed. Maintain confidentiality of patient information and adhere to HIPAA regulations. #J-18808-Ljbffr
