- NBI/Police Clearance (most updated with 6 months validity)
- PSA Birth Certificate (for Municipal Birth Certificate, it must be authenticated by PSA.
If none, PSA receipt should be provided)
- SSS (ID, E1, E4, Website Printout, verification slip)
- PhilHealth (ID, MDR, PMRF)
- Pag-Ibig (ID, MDF, MTR)
- TIN (ID, Form 1902, 1904, 1905, 2305, 2316, verification slip)
- TOR/Diploma
- _No proof of Government Number for SSS, Philhealth, Pag-Ibig, and TIN will not be allowed and may result to moved or deferred your start date_

**NON-CRITICAL REQUIREMENTS**:

- Barangay Clearance
- Certificate of Employment
- ID Pictures with white background
- PSA Marriage Certificate (if married)
- PSA Birth Certification of Dependants
- Training/Seminar Certification (if any)
- Cedula
