DUTIES AND RESPONSIBILITIES
Under the direct functional supervision of the Patient Care Coordinator:
- Adhere to commitments made to employing organization (LPMC)
- Maintain integrity of the profession by upholding the values and ethics of the employing organization (LPMC)
- Assist the employing organization (LPMC) in planning, organizing programs for effective social service, community development, interacting with professional persons.
- Respect the privacy of clients and hold in confidence all information obtained in the cause of professional service.
- Coordinate with the professional to ensure fair and reasonable, considerate, and commensurate with the service perform and with due regard for the clients ability to pay.
- Conduct initial screening and evaluation of patients and families
- Do comprehensive psychosocial assessment of patients (Case worker/Study)
- Assist patients and families understand the illness and treatment options, as well as consequences of various treatment refusal as needed (case to case basis)
- Help patients/families adjust to hospital admission; possible role changes; exploring emotional/social responses to illness and treatment;
- Educate patients on the roles of health care team members; assist patients and families in communicating with one another and to members of health care team; assist in interpreting information;
- Assist in educating patients on the levels of health care (i.e.
acute, sub
- acute, home care); entitlements; community resources; and advance directives;
- Facilitate decision making on behalf of patients and families;
- Employ crisis Intervention as needed.
- Inform hospital staff on patient psychosocial issues;
- Assist the Medical Staff in coordinating patient discharge and continuity of care planning;
- Assist in promoting patient navigation services;
- Arrange for resources/funds to finance medications, durable medical equipment, and other needed service with the corresponding government agencies.
- Maintain and update case work and other pertinent data/records.
