Home Health Rn Scheduler

Responsibilities:

Efficiently coordinate and schedule nursing visits and other clinical services for home health patients, ensuring optimal coverage and continuity of care
Collaborate with nursing staff, physicians, and other healthcare providers to develop and implement patient care plans
Review patient medical records and history to understand the needs of each patient and to ensure appropriate scheduling and care delivery
Serve as a key point of contact for patients, families, and caregivers to address scheduling concerns, changes in care plans, and other inquiries
Maintain accurate and up-to-date scheduling records, patient information, and care plans in compliance with company policies and healthcare regulations
Address and resolve scheduling conflicts, last-minute changes, and other challenges in a timely and professional manner
Ensure all scheduling practices comply with relevant state and federal regulations, as well as company policies and procedures

Qualifications:

Must be a Registered Nurse
At least 1 year of Home Health scheduling, medical records review, and care coordination experience
Proficiency in reviewing and interpreting medical records
Strong organizational skills with the ability to manage multiple tasks and priorities
Familiarity with Home Health regulations
Excellent attention to detail
Available for an immediate start
Open to flexible scheduling, including night shifts
