**Position Summary**
Evolent Health is seeking United States Licensed RN Care Advisors to join our team.

As a member of a team of nurses, social workers, registered dieticians, physicians, pharmacists, and program coordinators, you will have the opportunity to make a profound impact on the lives of people living with complex and/or chronic conditions.
You will connect with your patients, online and on the phone, while in the hospital, or in the physicians office - essentially however and wherever the patient needs your assistance to improve their health, better understand their illness and coordinate their care.
**Qualifications**
- An active and unrestricted Nursing license in the Philippines and mainland US (Evolent will cover cost for US license endorsement or renewal to the following states:

- Strong clinical knowledge and direct clinical patient care experience for at least 2 years
- Minimum of 1 year experience with US healthcare with basic knowledge and understanding of medical coding and claims processing.
- Experience in managed care and health insurance is an advantage
- Familiar with electronic health records preferred
- Excellent written and oral communication skills
- Proficiency with PC-based software programs including Word, Excel and Outlook
- Strong critical thinking, analytical, research and organizational skills
- Ability to work remotely and independently
- Hours of Training and Operations: US Business Hours
- Trainees will be required to maintain regular attendance and successfully complete the required training and demonstrate proficiency of the material at the end of training.
- Typing speed of 35 WPM
**Roles & Responsibilities**
- RN Care Advisor (CA) will connect with the patients and their care team on the phone when appropriate to improve care.
- Along with other members of the Care Management team, conduct comprehensive assessments that include the medical, behavioral, pharmaceutical and social needs of the patient, identify gaps in care and barriers to attaining improved health.
- Based on this assessment, and in conjunction with the patient, the patients physician and other members of the population health team, create and implement a care plan that will address the identified needs, remove the barriers and improve the health of the patient.
- Coordinate care by serving as the contact point, advocate and resource for the patient, their family and their physician, building effective relationships through trust, respect and communication.
- In close collaboration with the patient, primary care provider and care team you will continually assesses the patients knowledge of their clinical condition(s) and provide education and self-management support based on the patients unique learning style.
- Measure, improve and maintain quality outcomes (clinical, financial, and functional) for individual patients and the population served.
**Compensation & Benefits**
- Earn as much as 85,000 up to 95,000 per month
- HMO coverage includes you and a dependent.
